Define Hub and Spoke Model of Care

Hub: Organization that provides leadership and Opioid Use Disorder treatment expertise. They are the treatment provider that will help to stabilize their clients if the need arises. They offer a higher level of care, with additional support services to ensure their clients can be successful.

Spokes: The spokes are community partners that work with the hub sites to provide additional support services and case management. The spokes also help people reconnect with their own self-defined support system.

 

What is the purpose of this model?

The purpose of this model of care is to give consumers the support needed to be successful in recovery. The Hub is available if a client relapses or is on the verge of relapsing and needs extra support services. Once stabilized, the consumer can be transferred to a Spoke, to get both their medical and behavioral health medication needs met, while also being offered case management services if needed for additional resources. If the client feels that they need more support, the Spoke can facilitate a transfer back into the Hub.

 

Benefits of enrolling into this program:

  1. You will be able to go to one provider for your medication needs, instead of several.
  2. You will have a case manager to assist you with your individualized need.
    • For example, if you need help filling out an application for a job or for health insurance, they will sit with you and help
  3. You will be informed of any upcoming events and/or other local resources.
  4. If you need extra support or on the verge of a relapse/have relapsed, you will have easy access to higher level of care.
